Tarieven

Deze tarieven gelden voor patiënten van zorgverzekeraars waar wij geen overeenkomst mee hebben. Je ontvangt hiervan zelf de factuur. Je ontvangt ook zelf de factuur indien je niet verzekerd bent of indien jouw verzekeringspolis niet toereikend is.

Active Health Center Fysiotherapie heeft met alle verzekeraars een contract afgesloten behalve met Salland en Eucare-Aevitae. Ben je verzekerd bij een van deze verzekeraars dan kan je uiteraard wel bij Active Health Center terecht. Salland en Eucare-Aevitae vergoeden dan 70% van het bedrag.

De gereserveerde tijd voor een afspraak wordt niet in rekening gebracht indien de afspraak ten minste 24 uur voorafgaand aan het geplande tijdstip wordt geannuleerd. Wij adviseren u om de annulering per e-mail door te geven via ypenburg@activehealthcenter.nu, zodat voor beide partijen een aantoonbaar bewijs van afmelding aanwezig is.

Bij annulering binnen 24 uur voorafgaand aan de afspraak worden de kosten volledig in rekening gebracht, aangezien het binnen deze termijn doorgaans niet mogelijk is de vrijgekomen afspraak opnieuw in te plannen, waardoor inkomsten worden misgelopen..

Onze betalingsvoorwaarden zijn gedeponeerd bij de Arr. Te Utrecht onder nr.321/92.

These rates apply to patients with health insurers we do not have a contract with. You will receive the invoice for these insurers yourself. You will also receive the invoice if you are not insured or if your insurance policy is insufficient.

Active Health Center Physiotherapy has contracts with all insurers except Salland and Eucare-Aevitae. If you have insurance with one of these insurers, you can, of course, visit Active Health Center. Salland and Eucare-Aevitae will then reimburse 70% of the amount.

The reserved time for an appointment will not be charged if the appointment is cancelled at least 24 hours in advance of the scheduled time. We advise you to submit cancellations by email via ypenburg@activehealthcenter.nu, so that both parties have verifiable proof of the cancellation.

If the appointment is cancelled within 24 hours of the scheduled time, the full costs will be charged, as it is often not possible to rebook the released time slot at such short notice, resulting in lost revenue.

Our payment terms and conditions are filed with the Arr. (District Court) of Utrecht under number 321/92.
